New AppleCare Protection Plan
replaces the AppleCare Extended Service program in the US and Canada. The program is described here and here is a FAQ. It basically extends the original limited warranty to three years for both telephone support and service repairs and provides a CD with diagnostic tools. The cost ranges from US$150 for an iMac to US$300 for a PowerBook which is less than I paid in January for two years of AppleCare for my PowerBook 2400c. If 2 years of old AppleCare was bought on or after September 25th, you will be converted automatically. If AppleCare was bought before that (including a one-year plan, I guess), Apple will offer an upgrade presumably by mail.
